*Below are our terms and conditions regarding our monthly membership. The Urban Muse monthly membership includes several benefits:

• Monthly treatment is to be redeemed within the month you pay dues. (within 30 days after you pay for your membership).

• Membership treatments not used in the month will roll over and can be used for 12 months after the charged date.

• You may use one rollover per month.

• Any scheduled services must be canceled 24 hours prior, or the appointment is considered a used treatment.

• Membership and benefits are non-transferable and non-refundable.

• Your credit card is on file, and each month your credit card will be charged the amount recorded on the same date; "monthly dues date."

• Your credit card and email address are required for participation in our membership wellness program.

• This agreement may be terminated after 90 days by a member, provided that at least thirty days' notice is provided in advance of your monthly dues date.

• As of the effective date of a cancellation, any unused rollovers expire.

• The Urban Muse reserves the right to modify the terms and conditions of the membership wellness program upon advance prior written notice. memberships are a minimum of three months.

• Monthly memberships are renewed on an annual basis. Membership pricing is subject to change.
